Fingerprint cards are available and can be completed by most local law enforcement agencies.

To request fingerprint cards necessary for the criminal background check:

  • Email request with your address to , OR
  • If you prefer to use another facility's fingerprint cards, please write in the "Reason Fingerprinted" space "ALC 34-24-70" and under ORI number write "AL920049Z"

The average time for obtaining a license is six to eight weeks, depending upon various factors such as how quickly medical schools/postgraduate training programs provide primary source verification, how quickly the criminal background check results are returned, etc.

Once your certificate of qualification has been issued, you will need to apply to the Commission for a license using this form (also includes ACSC form which is only required if you require a registration to prescribe controlled substances).

If your application is being considered in the months of August - December, use this form, and choose whether you want the license to be issued immediately or on Jan. 1.



Limited certificate of qualification and license (license number L.####

Board of Medical Examiners' Administrative Rules 540-X-3-.14 through .16 establish qualifications and procedures for limited licensure in Alabama for:

  • Individuals in approved residency training programs (license numbers L.####R [resident], L.####F [fellowship])
  • Visiting professors, distinguished professors and specialty professors at medical colleges (license numbers L.###SP [specialty professor], L.####DP [distinguished professor], L.####VP [visiting professor])
  • Individuals employed by state mental and penal institutions (license number L.####SI [state institution])

There are specific requirements that must be met prior to the issuance of a limited license.  Limited licenses are issued for one year only.  Physicians must apply to the Board to renew their limited certificates of qualification by January 1 of each year.

Retired senior volunteer certificate of qualification and license (license number RSV.###)

Section 34-24-75.1 of the Code of Alabama authorizes the Board to issue certificates of qualification to physicians meeting the requirement for participation in the Retired Senior Volunteer Program (Volunteer Medical Professional Act (Ala. Code 6-5-660, et. seq. - scroll down to 6-5-660), without cost to the applicant.

The applicant is required to perform at least 100 hours of voluntary service annually and limit his or her practice to the confines of an established free medical clinic or other nonprofit organization or facilities in Alabama.

Administrative Rule 540-X-3-.23 provides the mechanism for application and issuance of the RSVP license. An application form and the form for the certification of the free medical clinic are available below.

Please note that physicians issued a license under the RSVP program remain subject to continuing medical education requirements.

Once your RSV certificate of qualification is issued, you will need to apply to the Commission for a license using this form (also includes ACSC form which is only required if you require a registration to prescribe controlled substances).

If your application is being considered in the months of August - December, use this form, and choose whether you want the license to be issued immediately or on Jan. 1. 



Special purpose certificate of qualification (license number SP.###)

Please review Board Rules, Chapter 540-X-16, the Practice of Medicine Across State Lines

  • special purpose licensees are limited solely to the practice of medicine or osteopathy across state lines (may not come into Alabama and practice)
  • qualifications for a special purpose certificate of qualification
    • - applicant's principal practice location and license is in a state/territory whose laws permit or allow issuance of a license to practice medicine across state lines or a similar license (see list)
    • - applicant holds a full and unrestricted license in any and all states in which the applicant is licensed
    • - applicant has not had any previous disciplinary action or other action taken by any state or licensing jurisdiction (the Board may waive this requirement if it finds the disciplinary action does not indicate that the physician is a potential threat to the public
  • Initial license fee is $175.00

Application for Special Purpose Certificate of Qualification

Once the SP certificate of qualification is issued, you will need to apply to the Commission for a license using this form (also includes ACSC form which is only required if you require a registration to prescribe controlled substances).

If your application is being considered in the months of August - December, use this form, and choose whether you want the license to be issued immediately or on Jan. 1. 



Medical Licensure Commission applications

  • Full, limited, RSV, SP, license application (Jan-July) (also includes ACSC form which is only required if you require a registration to prescribe controlled substances)
  • Full, limited, RSV, SP, license application (Aug-Dec) (also includes ACSC form which is only required if you require a registration to prescribe controlled substances) (use this form if your application is being considered during the months of Aug-Dec; you can choose to have your license issued immediately or on Jan. 1)



License renewals

  • All licenses are issued for one calendar year and expire on December 31
  • Renewal period for licenses is Oct 1 - Dec 31 (grace period with additional charge through Jan 31)
  • License renewal reminder postcards are sent to address of record beginning in October
  • Online renewals are available and encouraged
  • Be sure the information provided is accurate
  • If you do not wish to renew, request "Active-Non Renewal by Request" status
