How to use the licensee search

  • Enter last name and first name OR license number only. Partial entries are accepted.
  • Check “I’m not a robot."
  • To filter results, begin typing in “Filters” section under each column heading.
  • Click the printer icon in the “Details” column to view/print license information.

What information is provided

  • Name, license number, license issue date, license expiration date, medical school, medical school attendance dates.
  • ACSC/QACSC (controlled substances certificate) number, date issued, date of expiration.
  • Practice type and location (provided by the licensee).
  • CRNPs/CNMs in collaborative practice with and PAs/AAs registered to physicians.
  • Disciplinary actions are indicated by a “yes” next to “Public file” and downloadable.  Actions against physicians are a permanent record.

What information is not provided

  • Some information is confidential pursuant to state law:
    • Patient complaints.
    • Malpractice case information.
    • Investigations.
  • Full addresses and birth dates are not provided for security reasons.

Is the information provided through the licensee search considered primary source verification?

  • The information provided through the licensee search is primary source verified with the exception of practice specialty and location of practice.
  • Information provided through the licensee search is updated on a current basis in real time.

  • Full physician license:

  • Assistant to physician license:

  • Limited physician license (limited to the confines of the postgraduate training program, medical school or state institution):
