Procedure for name and/or address change

Address change

Alabama law requires licensees to notify the Commission of an address change within 15 days
Please use the change of address form
Duplicate registration certificates will be issued for the new location upon request
There is no charge for this service

Name change

Submit an application for name change
Provide the legal document verifying the name change
New license and ACSC certificates will be issued with the new name (no charge)
If you would like a new wall certificate, please use the replacement wall certificate request ($25 charge)
