License status options
- Inactive status - if you do not practice in Alabama and do not wish to renew your
license:
- - disregard renewal notices
- - on Feb. 1 (after renewal grace period) non-renewed licenses become Inactive-Failed to Renew
- OR
- - request "Active-Non Renewal by Request" status; you will not receive renewal notices and your ACSC will also be placed on non renewal status
- License reinstatement
- - to reinstate an inactive license, you should fill out the reinstatement application request
- - the cost to reinstate is up to $850, depending on when you were last active
- - 25 Category 1 or equivalent CME credits in the 12 months preceding the application are required
- ACSC reinstatement
- to reinstate an inactive ACSC, you should fill out the ACSC reinstatement application request
- the cost to reinstate is the current annual fee for an ACSC
ACSC/QACSC
- Inactive status - if you
do not handle controlled substances in Alabama and do not wish to renew your
(Q)ACSC:- - disregard renewal notices
- - on Jan. 1 all non-renewed (Q)ACSCs become Inactive - Failed to Renew
- (Q)ACSC reinstatement
- - to reinstate an inactive (Q)ACSC, you should fill out the (Q)ACSC reinstatement application request
- - the cost to reinstate is the current annual fee for a QACSC
