Duplicate License/ACSC Receipt and Wall Certificate If your annual license receipt has been destroyed or lost and you wish to receive a new one, please contact the Medical Licensure Commission at the telephone, address or e-mail listed on the  Contact Information page.  Please be advised that the Commission cannot fax copies of the license but will mail the replacement to the physician's address of record.  There is no fee for this service. For a replacement Alabama Controlled Substances Certificate, please contact the Board of Medical Examiners at the telephone, address or e-mail listed on the  Contact Information page  The Board cannot fax copies of the ACSC but will mail the replacement to the physician's address of record.  There is no fee for this service. If your wall certificate has been destroyed or lost, you may submit an Application for Replacement of Lost or Destroyed License to Practice Medicine, along with a $25.00 check, to the Medical Licensure Commission, PO Box 887, Montgomery AL  36101.  A notarized affidavit must accompany the application as to how and when the license was lost or destroyed.
