License and Alabama Controlled Substances Certificate (ACSC) Status Options

License:

If you do not live or practice in Alabama and do not wish to renew your license, disregard any renewal notices you receive, and your license will automatically revert to "Inactive-Failed to Renew" status on February 1.  This is a non-derogatory status.  If you choose to apply for reinstatement of your license at a later date, you should contact the Medical Licensure Commission by mail, facsimile or e-mail for a reinstatement application.  To reinstate, there is a reinstatement fee of up to $850, depending on when you were last Active.  You also must request and submit a reinstatement application along with documentation of having acquired 12 AMA PRA Category 1 ™ continuing medical education credits. Beginning January 1, 2011, the requirement for continuing medical education for reinstatement will increase to 25 Category 1 or equivalent credits earned in the 12 months preceding the application.

ACSC:

If you do not live or practice in Alabama, or if you practice in Alabama but do not write any prescriptions for controlled substances, you do not need to apply for or renew your Alabama Controlled Substances Certificate (ACSC). If you do not wish to renew your ACSC, simply disregard any renewal notices you may receive, and its status will revert to "Inactive" on Jan. 1. If you should return to Alabama to practice and will be writing controlled substances prescriptions, you may apply for the ACSC at that time by completing the application and paying the current certificate fee.
