Alabama Controlled Substances Certificate (MDs and DOs)

  • Initial ACSC applications are provided to physicians after their license applications are complete and certificates of qualification are issued.
  • ACSCs are renewed annually beginning in October. All ACSCs expire on December 31 of each year.
  • There is no "grace period" for renewing an ACSC. ACSC holders will be sent a notification in October with the procedure for renewal.

Qualified Alabama Controlled Substances Certificate (PAs)

  • Physician Assistants may apply for a Qualified Alabama Controlled Substances Certificate conferring prescriptive privileges in Schedules III, IV and V.
  • QACSCs are renewed annually and expire on December 31 of each year.
  • Application forms are sent to P.A.s upon their initial licensure and registration.
  • Please see the schedule of fees for current initial and renewal fee amounts.
  • QACSC holders must also register with the Drug Enforcement Administration.

Renewals

  • ACSCs and QACSCs expire on December 31 of each year
  • There is no grace period for renewing the ACSC/QACSC
  • Renewal notices are sent out along with license renewal notices in October of each year


Reinstatement

DEA registration

  • Both an ACSC/QACSC and a DEA registration are required to order, dispense, prescribe or otherwise distribute controlled substances.
  • DEA registrations are issued by the Drug Enforcement Agency.
