Physician and PA Search Please Read Before Entering Searchable Database Below Information other than the licensure and public file information provided through this search (e. g., malpractice cases, patient complaints, investigations) is confidential under state law and may not be released without the express written consent of the licensee
To search for a physician or physician assistant, enter the database below. Search by last name only, last and first name, first three letters of last name or license number.

The results page contains the public information concerning that individual's license (e.g., license number and status, issue date, expiration date, medical school, public actions).

Public and disciplinary actions are indicated by a "Yes" in the "Public File" field of the results page. If public action is indicated, a .pdf file of the documents will be accessible through a link at the bottom of the results page.


Note concerning Alabama Controlled Substances Certificate (ACSC) information:  At this time, our system allows us only to note whether or the licensee holds an ACSC, indicated by "yes" or "no."  It does not allow us to indicate a restricted ACSC.  If the ACSC is restricted, there will be a link to the public document explaining the restriction.  Therefore, if a physician's search results indicate ACSC "yes" but there is also a "yes" in the Public Actions field, you should review the public document to determine if there is a restriction on the ACSC.

Database Requests

For electronic databases (multiple names, addresses, etc.), please provide your name, organization name, address, telephone and facsimile along with your specific request to cmartin@albme.org. You will receive an invoice via return email and payment is required prior to the shipment of data (we do not accept credit cards). The cost is $.10 per record/field (e.g., first name, last name, street, city, state, etc.) for the first ten records and an additional $.01 per record over ten or a $100.00 minimum (whichever is greater).
