Important information
- The link below, "Proceed to Secure Login", will take you to the secure login page for the online renewals.
- Once you reach the login screen, it is important that you DO NOT use the back or forward buttons on your browser bar.
- This year you will only need your last name and the last four digits of your Social Security Number to login to the renewal site.
- After renewing your license, you will be asked if you want to renew any other licenses/registrations (ACSC, collaborative practice, etc.).
- If you want to renew by sending a check, see Manual Forms.
Late fee and expiration notices
- A $100.00 late fee will go into effect at midnight (Central Time) Dec. 31, 2011, for all MD, DO and limited licenses that have not been renewed.
- The $100.00 late fee will automatically be added when processing your renewal online.
- Any manual renewal forms that are postmarked after Dec. 31, 2011, must add the $100.00 late fee to the total or the renewal form will not be processed and will be returned.
Expiration notice for MD and DO physicians
- All licenses expire at midnight (Central Time) on Dec. 31, 2011, if they have not been renewed.
- Medical licenses (MD and DO) have a 31 day grace period and the physician can continue practicing until Jan. 31, 2012.
- ACSCs (Alabama Controlled Substance Certificates) DO NOT HAVE A GRACE PERIOD and expire Midnight Dec. 31, 2011. You may not write any controlled substance prescriptions until the Board processes your renewal in our office.
Expiration notice for limited license physicians
- All limited licensees must renew their Certificates of Qualification (COQ) each year.
- If you did not renew your COQ (and pay the COQ renewal fee) your license will expire at midnight (Central Time) on Dec. 31, 2011.
- There is NO GRACE PERIOD, and you can no longer practice medicine until you reapply for a limited license.
- You should contact your program coordinator for assistance in reapplying for a limited license.
- Limited licensees that have renewed their COQs do have a 31 day grace period to renew the limited license, and the physician can continue working until Jan. 31, 2012.
- ACSCs (Alabama Controlled Substance Certificates) DO NOT HAVE A GRACE PERIOD and expire at midnight Dec. 31, 2011. You may not write any controlled substance prescriptions until the Board processes your renewal in our office.
Expiration notice for PAs
- All licenses expire at midnight (Central Time) on Dec. 31, 2011, if they have not been renewed.
- PA (Physician Assistant) Licenses DO NOT HAVE A GRACE PERIOD and you can not practice as a PA until the Board has processed your renewal in our office.
- QACSCs (Qualified Alabama Controlled Substance Certificates) DO NOT HAVE A GRACE PERIOD and expire midnight Dec. 31, 2011. You may not write any controlled substance prescriptions until the Board processes your renewal in our office.
Expiration notice for AAs
- All licenses expire at midnight (Central Time) on Dec. 31, 2011, if they have not been renewed.
- AA (Anesthesiologist Assistant) licenses DO NOT HAVE A GRACE PERIOD and you can not practice as a AA until the Board has processed your renewal in our office.
Expiration notice for CP, SP or RSV
- All CP (Collaborative Practices), SP (Special Purpose) and RSV (Retired Senior Volunteer) licenses expire at midnight (Central Time) on Dec. 31, 2011, if they have not been renewed.
- These licenses DO NOT HAVE A GRACE PERIOD and you can not work under these licenses until the Board has processed your renewal in our office.
- If you are prompted to renew a CP registration and you do not think you have a CP, contact our office–if our records indicate a CP and it is not renewed or formally terminated by Dec. 31, it will be reported to the Board (online termination form available here)

Manual forms
- NOTICE: In you have a popup blocker (example: Google Toolbar) you must allow popups or add the renewal site as a trusted site in order to view and print the manual forms.
- In order to view and print a manual form, click the link below and login.
- Once you have logged in, click on the "Print Form and Mail Check" button.
- Then click on the "Print Form" button to the right of your license.
- Click the "Print Form" button for each license that you have to renew.
- Remember that you can answer the questions and update information on the form before printing.

Printing Receipts and Renewals

Renewing ACSC or QACSC
During the renewal of a Alabama Controlled Substance Certificate (ACSC) and a Qualified Alabama Controlled Substance Certificate (QACSC) you will be asked for your Federal DEA Number and Expiration Date -- please have this information available.

The online renewal site is secured by the use of HTTPS software. Any information entered for renewals is secure, and unavailable for retrieval by outside parties. When you click on the link below you will be taken to the secure site where you will be asked to enter your login information.
